When completing our online reimbursement form, you may upload electronic copies of your receipts. If you prefer, you may instead use our Reimbursement Form 2022-23
The UAHS PTO does not pay tax. If you did pay tax, the PTO will only reimburse you the amount prior to the tax. The tax is a contribution from you. If you need the Tx-ID number, please contact the Treasurer at [email protected].
